Making quality healthcare accessible to everyone is part of our ongoing commitment to the community. The patient financial assistance program is one of the many ways Berger helps patients fulfill their financial obligations for their healthcare services.
We are proud to offer several programs to patients in need of financial assistance.
Medicaid program eligibility assistance
There are several Medicaid programs available to Ohio residents. Our financial services representatives can assist you with the application and eligibility process for the Healthy Start and Healthy Families programs; Medicaid for the aged, blind and disabled; and Alien Emergency Medical Assistance. If you are found eligible for one of these programs, you may be covered for hospital care, provider services, prescriptions, mental health services, and vision and dental services.

- Visit the Ohio Department of Jobs and Family Services website to download an application.
Hospital Care Assurance Program (HCAP)
The Hospital Care Assurance Program (HCAP) is available to Ohio Residents who are not currently eligible for Medicaid and whose total gross family income is at or below the federal poverty guidelines. If you qualify for the HCAP program, you may be eligible for basic medical care without charge. The current HCAP guidelines are as follows:
- 2016 poverty level.
- Discount amount – 100 percent.
- PT RESP. 0 percent.
- For each additional person, add $4,180
We offer additional financial assistance programs to our patients who are not eligible for Medicaid or HCAP. Eligibility is based on family size and income up to 250 percent of the current federal poverty level. Individuals with extenuating circumstances will be considered for additional assistance programs.
Our financial services representatives are available to assist you in determining your eligibility for financial assistance with your hospital bills. To begin the application process you must complete a financial aid application. To obtain a financial aid application:
- Please visit the patient financial services (PFS) department at 1180 N. Court St., Circleville, OH 43113.
- Call 740.420.8020, 8 a.m. to 5 p.m. Monday through Friday, to speak with a PFS support specialist or to schedule an appointment.

We realize that medical bills can often be overwhelming and complicated to understand.

The patient is ultimately responsible for his or her hospital bill. We encourage you to confirm that your insurance covers Berger Hospital services prior to your visit by calling the phone number listed on your insurance card.

We accept most health plans available in central Ohio. But, before you seek care with us, please review the list below to make sure your plan is included. It's a good idea to also check with your insurance company, as plans and providers can change. What is covered and how much you will need to pay may also vary by plan.

Participating health insurance marketplace plans
At this time, Berger Health System is in-network for the following health insurance marketplace plans:
- Medical Mutual-Ohio Health HMO.
- Molina Marketplace.
For other plans purchased through the health insurance marketplace, Berger Health System is an out-of-network provider.
