Advance directives

The Berger care management team is available to assist patients and their families with advance directive documents that can provide vital healthcare information for a patient and their family. These may include:

Living will

A living will is a document that tells a person's doctor or other healthcare providers whether the patient wants life-sustaining treatments or procedures administered if he or she is in a terminal condition or a permanently unconscious state.

Healthcare durable power of attorney (HDPOA)

A durable power of attorney is a legal document that allows a person to appoint another person to make medical decisions for him or her if he or she should become temporarily or permanently unable to make those decisions.
