Patient medical records are confidential and shall not be released or disclosed except in accordance with federal, state, and Berger Health System regulations. Although the information contained within the medical record belongs to the patient, patient medical records are the property of Berger Health System. The Medical Records Department employs trained professionals to safeguard your medical information.

How Can I Obtain A Copy Of My Medical Record?
You can request your medical records either in person or in writing. If you request your medical records in writing, we will need your full name, date of birth, date of service being requested, your address, and phone number in case we need to contact you. If you would like your records sent directly to your provider please include their name, address, phone number, and fax number.

What Identification Is Required To Obtain My Medical Record?
Prior to the release of patient medical information, the requesting party shall be required to provide proof of identification by providing picture identification. If a request is received to send medical information to another provider for continuity of care proof of identification is not needed.
How Soon Can I Expect The Request To Release My Medical Record To Be Completed?
If you request medical information in person, we try our best to print the information while you wait. Depending on how much information is being requested, you may be asked to come back at a later date to pick up the copies. The medical information will be released within 1-5 business days.
Am I Able to Get A Copy Of A Family Member's Medical Record?
Only the patient may release their records. However, if you are the Power of Attorney (POA) or the executer of the patient's estate, you can request family member's records accompanied by a copy of the legal document giving you POA or executer rights. Patients can also authorize family members to pick up their records, but it must be in writing.
Do You Release Any Information Verbally?
The only information the Medical Records Department will release is the patient's date of admission and the date of discharge.
How Can I Obtain My Birth Certificate?
Question about birth certificates should be directed to the Office of Vital Statistics, Pickaway County, Ohio.
How Long Do You Keep A Medical Record?
Medical Records are kept according to Ohio statute. You will be contacted if your medical record request cannot be completed due to the retention period being expired.
How Do I Get A Copy Of My Radiology Film?
To obtain a copy of your X-Ray film, you must call the Radiology Department directly at 740-420-8248.
Am I Able To View My Medical Record?
Yes, you are able to view your medical record. You must provide a written request signed and dated. You can contact the Medical Records Department at 740-420-8237 to schedule an appointment. Please bring a photo ID to your scheduled appointment.
