Participating Insurance Plans
Berger Health System accepts most health plans available in central Ohio. It's a good idea to also check with your insurance company, as plans and providers can change. What is covered and how much you will need to pay also vary by plan.

Financial Assistance

The Patient Financial Assistance Program is one of many ways Berger helps patients fulfill their financial obligations for their healthcare services.Berger Health System is proud to offer several programs to patients in need of financial assistance.
Medicaid Program Eligibility Assistance
There are several Medicaid programs available to Ohio residents. The Financial Services Representatives at Berger Health System can assist you with the application and eligibility process for the Healthy Start and Healthy Families programs, Medicaid for the aged, blind and disabled, and Alien Emergency Medical Assistance. If you are found eligible for one of these programs, you may be covered for hospital care, provider services, prescriptions, mental health services and vision and dental services.To obtain an application:

HCAP - Hospital Care Assurance Program
The Hospital Care Assurance Program (HCAP) is available to Ohio residents who are not currently eligible for Medicaid and whose total gross family income is at or below the federal poverty guidelines. If you qualify for the HCAP program, you may be eligible for basic medical care without charge. The current HCAP guidelines are as follows:

Charity Care Program
Berger Health System offers additional financial assistance programs to our patients who are not eligible for Medicaid or HCAP. Eligibility is based on family size and income up to 250% of the current federal poverty level. Individuals with extenuating circumstances will be considered for additional assistance programs.
Application Process
Financial Services Representatives are available to assist you in determining your eligibility for financial assistance with your hospital bills. To begin the application process you must complete a financial aid application. To obtain a financial aid application:
- Please visit the Patient Financial Services Department at 1180 North Court Street, Circleville, OH 43113.
- Call 740.420.8020, 8:00 a.m. - 5:00 p.m., Monday - Friday to speak with a Financial Services Representative.
- Call 740.420.8059 or 740.420.8529, 8:00 a.m. - 4:30 p.m., Monday - Friday to speak with a Financial Counselor or schedule an appointment.
- Print the Berger Health System Financial Aid Application and fill out the form. Send completed form, along with any attachments, to the Patient Financial Services Department at 600 North Pickaway Street, Circleville, OH 43113.
