Advanced Directives

Berger Health System staff is available to assist patients and their family members who may need help with advanced directive legal documents. To request assistance, please call 740.474.2126 or 888.477.2126 and ask for the Care Management Department, Monday through Friday, from 8 a.m. to 4 p.m.

Living Will

A living will is a document that tells a person's doctor or other healthcare providers whether the patient wants life-sustaining treatments or procedures administered if he or she is in a terminal condition or a permanently unconscious state.

Healthcare Durable Power of Attorney (HDPOA)

A durable power of attorney is a legal document that allows a person to appoint another person to make medical decisions for him or her if he or she should become temporarily or permanently unable to make those decisions.
