Patient Home Medication Lists

Berger Health System is committed to providing quality, safe and convenient care to our patients. Preventing medication-related errors should be a major focus of all who are involved in healthcare, including patients, physicians and nurses. To address this, Berger Health System practices the process of medication reconciliation. This is the process of reviewing a patient's current medications prior to ordering additional medications for the delivery of care. Dr. Gary Gillen, Berger's Vice President of Medical Affairs, states, "One of the most important pieces of information that will guide the diagnosis and therapy of a sick or injured person is the identification of the medications they are taking. Problems can be prevented when a new medication is added if we know the other medications that a person is taking."

"To deliver the safest and most quality care to you, nurses need to collect accurate information about the medications you are currently taking," said Connie Kelly, Berger's Chief Nursing Officer. "This information is used by the physicians to develop your plan of care while in the hospital or when accessing other services or testing at the hospital, to ensure nothing interferes with the medications that you are currently taking."

Berger Health System encourages EVERYONE to create and maintain a list of prescribed and over-the-counter medications you are currently taking before coming to the hospital or going to a doctor's appointment.

This medication list should include:

  1. Medication's name.
  2. Medication's dosage.
  3. How many times you take the medication each day.
  4. The way you take the medication; for example, by mouth, by injection, etc.
  5. The reason you take the medication.
  6. Any allergies or reactions you have from medications.
  7. All prescribed and over-the-counter medications you are currently on, including herbal, vitamin and dietary supplements.

Be sure to keep the medication list with you at all times in your purse or wallet. Have the list updated every time you have an encounter with a healthcare professional. If there are any changes to your medications, ASK them to update the list for you.
