Patient Financial Services
Berger Hospital is currently participating with the following insurance plans. Often times these plans will lease
their network to other plans. In most cases the logo of the primary network is present on the insurance card. If
you do not see your logo, or the insurance plan that you currently are enrolled in listed below, please contact the
Insurance Hotline at (740) 420-8524 for additional information.

Like any large industry, insurance has developed somewhat of a language of its own. Berger Health System has compiled
a glossary of Common Insurance Terms to help you better understand the
complexities of medical billing and insurance coverage.
We realize that medical billing is very complicated. Berger Health System has put together a list of
Frequently Asked Questions (FAQ) to help you avoid any unpleasant billing surprises
following your treatment.

The patient is ultimately responsible for his/her hospital bill. We encourage you to confirm that your insurance
covers Berger Health System services ahead of time by calling the phone number listed on your insurance card.

The Patient Financial Assistance Program is one of many ways Berger Health System helps patients fulfill their financial
obligations for their health-care services. Berger Health System is proud to offer several programs to patients in need
of financial assistance.
There are several Medicaid programs available to Ohio residents. The Financial Services Representatives at Berger Health
System can assist you with the application and eligibility process for the Healthy Start and Healthy Families programs,
Medicaid for the aged, blind, and disabled, and Alien Emergency Medical Assistance. If you are found eligible for one of
these programs, you may be covered for hospital care, physician services, prescriptions, mental health services, and vision
and dental services.
To obtain an application:
- Call Berger Health System Patient Financial Services at (740) 420-8020
- Call the Medicaid Consumer Hotline at 1-800-324-8680 (TTY users call: 1-800-292-3572)
- Visit the Ohio Department of Jobs and Family Services web site to download an
application.
The Hospital Care Assurance Program (HCAP) is available to Ohio Residents who are not currently eligible for Medicaid and
whose total gross family income is at or below the federal poverty guidelines. If you qualify for the HCAP program, you
may be eligible for basic medical care without charge. The current HCAP guidelines are as follows:

Berger Health System offers additional financial assistance programs to our patients who are not eligible for
Medicaid or HCAP. Eligibility is based on family size and income up to 250% of the current federal poverty level.
Individuals with extenuating circumstances will be considered for additional assistance programs.
Financial Services Representatives are available to assist you in determining your eligibility for financial
assistance with your hospital bills. To begin the application process you must complete a financial aid
application. To obtain a financial aid application:
- Please visit the Patient Financial Services Department at 1180 North Court Street, Circleville, Ohio 43113.
- Call (740) 420-8020, 8:00 a.m. - 5:00 p.m., Monday - Friday to speak with a Financial Services Representative.
- Call (740) 420-8059 or (740) 420-8529, 8:00 a.m. - 4:30 p.m., Monday - Friday to speak with a Financial Counselor
or schedule an appointment.
- Print the Berger Health System Financial Aid Application and fill out the form.
