Berger Opens Disease Management Clinic

Circleville, OH -- November 24, 2003 Berger Health System recently opened an outpatient Disease Management Clinic to offer specialized services that were unavailable within proximity of Pickaway County residents.

The Disease Management Clinic provides comprehensive care to patients who suffer from chronic diseases. The goal of the clinic is to decrease hospitalizations, prevent complications associated with chronic disease, and improve patients' quality of life.

Comprehensive care and self-management education is being offered for the following conditions: diabetes, asthma, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), hypertension, and high cholesterol. The clinic also offers management for patients taking the anticoagulant Coumadin and an individualized smoking cessation program.

"Disease Management is not an entirely new concept, but the approach Berger Health System is taking is quite unique to central Ohio," said Karlyn Nedd, Pharm.D., Coordinator for the Disease Management Clinic.

"Our healthcare team, consisting of a registered nurse, clinical pharmacist, respiratory therapist, dietician, certified diabetes educator, and other professional disciplines, strives to provide care for all aspects of the patient's disease -- all at one location. We work directly with the patient's physician to provide education to the patient about his/her disease and medications, self-management techniques, evaluation, and follow-up."

Dr. Katherine McClanahan, an internal medicine physician, is serving as Medical Director for the clinic. "The clinic is an excellent resource for physicians, including myself, to refer patients for education and management of their condition in between their regular doctor visits," noted McClanahan.
