Medical Records
The Medical Records Department is where all Berger Health System patient charts are housed. The purpose of this
department is to maintain confidentiality and security of medical information. They provide the release of
medical information to patients and physicians. Patients are asked to sign a consent form for the release of
protected health information.
To request a copy of your medical record, call 740.420.8237. The office is open from 8:00 a.m. to 4:30 p.m.,
Monday-Friday. Walk-ins are welcome; however, it is recommended that you call ahead. When patients arrive at the
Medical Records Department to pick up their records, they will be asked to show proper identification before the
records will be released to them. A minimum of 24-hour notice is requested for obtaining records covering
multiple dates of service.
